Is There Really One “Biological Age”?

A sweeping new study of women suggests that aging is not one clock ticking steadily forward—it is a collection of highly personal changes shaped by genes, environment, and even the time of day.
Is there really one biological age?

The promise is irresistible: Send in a blood or saliva sample, and a laboratory will tell you not merely how old you are, but how old your body really is. A 68-year-old might be thrilled to learn that her “biological age” is 54, or alarmed to be told it is 75.

Biological-age tests have become a centerpiece of the longevity industry. Their appeal rests on the idea that aging leaves a measurable signature and that one number can summarize how quickly the body is declining. But a major new study published in Science suggests that human aging is far more individual, dynamic, and context-dependent than a single score implies.

The central finding: Two healthy women of exactly the same chronological age can be moving in markedly different, and sometimes opposite molecular directions.

What Science Tracked

Researchers led by King’s College London repeatedly studied 335 women in the TwinsUK cohort over eight years. Rather than taking one sample and comparing older people with younger people, they followed the same women through time.

The team measured activity across roughly 16,000 genes and hundreds of metabolites, small molecules produced as the body processes food, medications, hormones, and other substances. Of those, 5,061 genes and 181 metabolites changed over the study period.

At the group level, scientists could identify broad patterns associated with getting older. But individual women often departed from those averages. Some molecular markers increased in one participant while decreasing in another. In other words, the population’s average aging pattern did not reliably describe every person within it.

Aging is Not One System

Genetics and biological age testing

The body does not age as a single unit. The cardiovascular system, immune system, muscles, brain, kidneys, bones, and metabolism can change at different rates. A woman may have excellent cardiovascular fitness but declining bone density. Another may remain physically strong while insulin resistance or inflammation quietly increases.

That helps explain why one biological-age number can be misleading. A composite score may average many signals together, hiding areas of exceptional health as well as areas that deserve attention. It may also change depending on which markers a company measures and which algorithm it uses.

Genes matter, but they are not the whole story

Because the participants included identical and fraternal twins, the researchers could examine genetic influence. Identical twins tended to have more similar gene-expression patterns than fraternal twins, confirming that inherited biology shapes aging trajectories.

Yet even identical twins did not age in perfect molecular lockstep. Environmental exposures, health conditions, medications, diet, stress, and other lived experiences also influence the body’s molecular profile. The new research reinforces a hopeful point: Genes help write the opening chapters, but they do not dictate every page.

Even season and time of day changed the picture

One of the study’s most revealing findings was how strongly timing influenced the measurements. Researchers found widespread daily and seasonal patterns among genes and metabolites. A blood sample taken in the morning may not look identical to one taken later in the day. A sample collected in winter may differ from one collected in summer.

That does not make molecular testing useless. It means testing needs careful standardization and interpretation. If a result shifts because of sleep, infection, a medication, the season, or the hour of collection, consumers should be cautious about treating a single measurement as a permanent verdict on how well they are aging.

Environmental change showed up in the blood

The researchers also observed changing blood levels of PFAS, the persistent industrial chemicals often called “forever chemicals.” Those levels declined over time in some participants, possibly reflecting reductions in exposure following regulation and changes in consumer products.

This is a reminder that aging occurs within an environment. Public policy, air and water quality, workplace exposures, and everyday products may leave biological traces. Healthspan is not shaped solely by personal discipline or an expensive supplement regimen.

What does “biological age” actually mean?

Biological age tests

There is no single universally accepted biological-age test. Different clocks use different inputs and answer slightly different questions:

  • Epigenetic clocks examine chemical tags on DNA that influence gene activity.
  • Blood-based clocks combine markers associated with inflammation, metabolism, organ function, or mortality risk.
  • Proteomic and metabolomic clocks analyze patterns in proteins or metabolic compounds.
  • Functional measures use strength, walking speed, balance, aerobic fitness, cognition, or other real-world capabilities.

Two tests can therefore give the same woman two different ages without either laboratory making a calculation error. They may be measuring different biological systems, using different comparison populations, or defining “aging” differently.

What the new study does and does not prove

The study is unusually valuable because it followed the same people repeatedly instead of drawing conclusions from a single snapshot. It also combined gene activity and metabolite measurements, providing a more detailed view of change over time.

But it has limitations. The cohort was relatively small, included only women, and relied on blood samples. Blood can reveal important systemic changes, but it cannot fully describe what is happening inside every organ. The participants were also twins from a research cohort, so the findings need confirmation in larger and more diverse populations.

Most important, the researchers did not conclude that all aging clocks are worthless. Broad biological-age scores may still help predict disease or mortality at a population level. The study shows why those scores should not be confused with a complete, fixed, personally precise measurement of the whole body.

Before buying a biological-age test

A consumer test can be interesting, and repeated measurements under consistent conditions may eventually prove more useful than a one-time result. Before spending the money, ask:

  • Has the test been validated in independent, peer-reviewed research?
  • Does it predict a meaningful health outcome, or merely correlate with chronological age?
  • Were women my age and background adequately represented in the reference population?
  • How much can the result vary with illness, medication, sleep, season, or collection time?
  • Would a different score lead to a medically useful action—or only anxiety and another purchase?
  • How will the company store, use, or share my genetic and health data?

Do not let one proprietary score overrule established measures your clinician can interpret like blood pressure, cholesterol, glucose, bone density, body composition, strength, balance, fitness, sleep, and cognitive or functional changes.

A better way to think about aging

Instead of asking, “How old is my body?” it may be more useful to ask, “Which parts of my health are holding steady, which are improving, and which need attention?” That turns aging from a verdict into a set of observable trajectories.

A woman’s grip strength can improve even as birthdays accumulate. Blood pressure can fall. Fitness can rise. Bone loss can be detected and treated. Sleep can recover. None of these changes makes chronological age irrelevant, but they demonstrate that important elements of aging remain modifiable.

The new study’s deeper message fits what many women already know from experience: People do not become biologically interchangeable simply because they share a birthday. Aging is neither one clock nor one road. It is a changing map, and increasingly, science is learning how to read it. A biological-age score may offer one piece of information, but it is not your expiration date, your destiny, or a complete measure of your health. The most meaningful picture is multidimensional and tracked over time.
